Patient writes on her FFT Card "I find all the staff professional and polite but I don’t think the practice is functioning well. I feel that without prompting and chasing there is very little communication or follow up care. I do feel that the practice would benefit from a process review on ensuring preventative proactive care and close the loops on engagement with patients. Just to reiterate no issue with any staff or gps when I am interacting with them."
